What is Form 25C Under North Carolina Worker’s Compensation Law?

The employer or insurance company may hire a rehabilitation professional to monitor an injured workers medical treatment. The rehabilitation professional may attend medical treatment of the injured worker and discuss the medical condition and treatment of the injured worker with the treating physician. The rehabilitation professional is usually a nurse or other medical professional who reports back to the insurance company or employer on the medical condition and treatment of the injured worker.
North Carolina law usually requires the injured worker to sign the Form 25C authorization. Failure to sign this form could result in termination of benefits to the injured worker.
The rehabilitation professional is presumed to be an unbiased third party who reports the injured worker’s medical facts to the insurance company or employer. However, the insurance company or employer usually pays the nurse case manager or rehabilitation professional. If you believe your nurse case manager is biased against you then you should immediately discuss this with your workers’ compensation attorney. The injured worker or his/her employer is entitled to a copy of all reports.
